Understanding Evergreen Life

  • What is Evergreen Life
    Your NHS - all in one place with Evergreen Life. Evergreen Life helps you put all your healthcare on one page. You can:
    • Book and cancel appointments,
    • Order your repeat prescriptions,
    • Send messages to your practice (if your GP practice has allowed that function)
    • View your medical record
    • You can arrange your page as you like. You can also add gadgets to your page that help you keep up to date with the health issues that interest you. That can be something like Asthma UK if that were relevant to you; or NHS Choices with its huge store of information; or Patient Opinion where you can comment on your health care and help improve it.
  • How does Evergreen Life work?
    Evergreen Life links securely to your doctor's computer system to offer the services. By acting as a gateway beetween the internet and your doctor's system it improves security, presents the information in an easy to use form and adds extra information to help you understand what you are seeing.

    Evergreen Life doesn't store any of your medical information so what you see is always secure and up to date.
  • Understand more of what you read
    Wherever you see a blue button, it will take you directly to relevant information. For instance, a technical term in your record will be explained if you click on the blue info-button. Or a test result. Or a drug that you are taking. Evergreen Life wants to help you understand what you are reading so that you can take as much control over your health and care as you want. Most, but not all technical terms are covered.
  • Getting the most from Evergreen Life
    1. Add a Gadget: If you go to Settings and then New Gadgets, you can pick from a list of gadgets that you can place on your home page. They can help you keep up to date with areas of health and illness that matter to you or your family. They can also help you interact better with parts of the NHS or other relevant services. For instance, by commenting on health and social care services, telling the NHS about any side-effects of your medications, or helping you get in touch with Citizens Advice.
    2. Save time: You can book and cancel your appointments, order prescriptions, check your test results and contact your practice.
    3. Safer care: Check if your records are accurate, share data with members of your family or health professionals and carry your health information whereever you want.

  • Is Evergreen Life a free service?
    Yes, Evergreen Life is provided free of charge to UK practices and their patients.

Evergreen Life App

  • What devices can I download the Evergreen Life app on to?
    The Evergreen Life app can be downloaded to devices running iOS 9 and above and all Android devices running Jelly Bean, Kit Kat and Lollipop. Please see below for a full list of Apple devices:

  • Where can I download the app from?
    The Evergreen Life app is available from the Apple store, Google Play store and the iTunes store.
  • How do I change my password?
    To change your password simply log on to the app, click on to the menu and enter the 'Security Settings'. Here you will be able to change your password as well as set password prompts and recovery emails.
  • What happens to my medication reminders if I go abroad or the clocks go back/forward?
    The app will adjust to new Time Zones or Daylight Saving Time adjustments as long as you close down the app completely and re-login to the app following a time change or adjustment.

    You should close down the app completely and re-login to the app when you travel into a new time zone to allow the medication diary and reminders to synchronise with the new time zone. If the time zone you have moved to means that you skip over a scheduled medication time slot in the app you will need to go back and record this manually.

  • How can I share my health record?
    This is your information, you own it and you are in control of it.

    There are benefits to sharing your data, when you choose to. We know sometimes that the health professionals you meet don't always have your up-to-date health information and this can result in delays to your care or the same questions being asked over and over again.

    Wouldn't it be great if you always had your health record with you at every appointment or in an emergency at the touch of a button?

    We know sometimes that people with busy lives who care for say, an older parent or grandparent; can experience a lot of extra worry and stress when they cannot access information to support their loved one in their care.

    Wouldn't it be great if you could tell if your loved one had managed to take those important lunch-time medications at the touch of a button?

    We know sometimes that people want to be positively engaged in their fitness and long-term health and want to say as fit as possible for as long as possible.

    Wouldn't it be great if you could start sharing your health goals with your doctor and get expert advice on your health and fitness in one place at the touch of a button?

    Sharing your health record could not be simpler.

    Just go to Account and Security, select the Share & Care feature and enter the email address of the person you wish to share your Personal Health Record with. The app will send them a link to download the app and they can accept your sharing request and switch to view your profile. If they already have an Evergreen Life Personal Health Record account then your sharing request will automatically appear in the Share & Care feature of their app. Don't forget you are completely in control of your health record so if at any time you wish to stop sharing your record this is possible at the touch of a button.
  • I am a healthcare professional. How can I access my patient’s health record?
    People can share their Personal Health Records with any Health Professional they choose to. All they need is your email address, which they enter in the Share & Care feature of the app. Evergreen Life will then send you an invite to download the app where you will see that your patient has sent you a sharing request. Simply accept the sharing request and you will be able to switch to their profile and see their Personal Health Record and any of their observations such as blood pressure, peak expiratory flow or blood glucose results.
  • How secure is my data?
    We will help keep your personal health record data safe, secure and private.
    • Your personal health record is password protected to prevent unauthorised access by anyone else.
    • The data that is entered, transmitted and stored is encrypted to prevent any unauthorised use of the data.
    • The data is stored in secure servers and data centers. These servers or data centers are located within the European Economic Area and the data is always treated and stored securely.
    The electronic transmission of data over the internet is never completely secure but Evergreen Life endeavor to make the service as secure as possible and provide security similar to those provided by banking operators.

Login Help

  • Why doesn't my sign-in work?
    All of the sign-in information is case sensitive. So it may be that your caps lock key is set incorrectly.

    If you still can't sign-in we may be experiencing technical problems so please try again later. If you've done this and still cannot sign-in, please contact us and we will investigate further.
  • How do I change my password?
    There is an option to change your password on the My Account section of the Settings page once you have logged in.
  • I've forgotten my password
    You may reset your password by following the instructions on the Forgotten password page.
  • I'm having problems with cookies

    You need to enable cookies to use this system. We keep track of your user session using cookies, so your browser must accept them for the system to work correctly. We don't keep any information about you with these cookies, in fact we send a fresh one every time you return.

    To enable cookies use the following steps:

    Internet Explorer

    • Open Internet Explorer
    • Click the Tools button and then click Internet options
    • Click the Privacy tab and under Settings move the slider to the top to block all cookies or to the bottom to allow all cookies, and then click OK.

    Chrome

    • Select the Chrome menu
    • Select Settings
    • Near the bottom of the page, select Show advanced settings
    • In the Privacy section select Content settings
    • Select Allow local data to be set (recommended).
    • Select Done.

    Firefox

    • Go to the Firefox drop-down menu
    • Select Preferences
    • Click Privacy
    • Set Firefox will to Use custom settings for history
    • Check the box next to Accept cookies from sites to enable cookies

  • Can I sign-in from any computer?
    Yes. You can use our online service from any computer. You don't need to re-register. Just be sure to sign-out when you have finished using the service!

  • Browser requirements

    In order for Evergreen Life to function correctly, your browser will need to have both Stylesheets and Javascript enabled.

    To enable Javascript:

    Internet Explorer

    • From the Tools menu, select Internet Options
    • Click the Security tab
    • Locate the Custom area
    • Click the Custom Level button
    • From the Security Settings dialog that opens, scroll through the options until you see Scripting
    • Check the Radio Buttons next to Enable Active Scripting
    • Click OK to accept scripting.

    Chrome

    • Click the Chrome menu icon on the browser toolbar
    • On the Settings page
    • Click the Show advanced settings link
    • Select Allow all sites to run JavaScript (recommended) in the JavaScript section

    FireFox

    • Click the Tools drop-down menu and select Options
    • Check the boxes next to Block pop-up windows, Load images automatically, and Enable JavaScript
    • Refresh your browser by right-clicking anywhere on the page and selecting Reload, or by using the Reload button in the toolbar
    • Stylesheets will be enabled by default on your browser
  • Is Evergreen Life a free service?
    Yes, Evergreen Life is provided free of charge to UK practices and their patients.

Creating an Account

  • Start using Evergreen Life
    To start using Evergreen Life you need to create your Evergreen Life account and link with your practice.
    1. Create your Evergreen Life Account: You will need to enter an Email address, your full Name, Password, select a security question and type in the answer on the Account Signup page

      Your Email & Password will be used as your login details. The email address, security question and answer will be needed for resetting your details if you ever forget your login details.
    2. Link to your practice: You need to ask the receptionist at your practice to register you with your practice and enable your access for online services. The receptionist will give you a registration letter which will have your Practice details, Account ID and Account Linkage Key. These details are required to link Evergreen Life with your practice account.

      After creating your Evergreen Life account using Step 1, the website will take you to a form for linking with your practice. All the details required to complete this form will be in your registration letter. Select your practice name from the drop down menu and then fill in all the details in the form which follows the drop down menu. Click on Link Account to complete linking. Your Evergreen Life account will be linked to your practice.
    3. Login to Evergreen Life and start using Evergreen Life
  • Linking with your practice after logging in
    The account creation form is followed by a linking form. If you don't have your registration letter with you then you can use the link option after logging into Evergreen Life once you have your registration letter with you.

Frequently Asked Questions

Registration and Account Creation

  • Why do I need to link with my practice?
    Evergreen Life works with your GP practice. When you register with your practice all your details are held in the practice clinical system with your unique id. Evergreen Life extracts this data and displays it to you. The connection between your Evergreen Life account and your practice account is made by using your account linkage key.
  • I am already registered for online services with another site. What should I do?
    If you are already registered for online services through another provider, you can ask reception at your GP to print you a registration for online services letter. The registration letter will have all details you need for linking your Evergreen Life account to your GP. You will need to take proof of your identity with you.
  • I have lost my registration letter. What should I do?
    You can ask reception at your GP to reprint your registration letter - you will need to take proof of your identity with you.
  • I am not registered for online services with my GP. What should I do?

    You can create your Evergreen Life account without being registered with your practice by going to the Account Signup Page.

    To make use of the service you need to register for online services with your practice. The receptionist at your practice will be able to help you with this. After registering for online services with your practice, the receptionist will give you a registration letter.

    The registration letter will have all details you need for linking your Evergreen Life account to your practice account.

  • I am not registered with a GP. What should I do?
    You can find your nearest GP on the NHS Find GP Services page.

Evergreen Life features

  • Why can I not book appointments?
    If you cannot see the appointments booking section, your practice may have disabled the service. Your practice controls access to this service. Please check with your practice.
  • Why can I not request medications?
    If you cannot see a section that allows you to request medication, your practice may have disabled the service. Your practice controls access to this service. Please check with your practice.
  • Why can I not send a secure message to my practice?
    If you cannot see a section for sending messages to your practice, your practice may have disabled the service. Your practice controls access to this service. Please check with your practice.
  • Why can I only book a fixed number of appointments?
    Your practice can decide on the maximum number of appointments you can book online. If you have reached that limit you will not be able to book any more appointments online.
  • Why are email verification and the security question important?
    If you forget your login details Evergreen Life can help you reset it. To make it more secure we will ask you to answer the security question you had selected during account creation and then send the reset code to your already verified email account. You will need this code if you have forgotten your password.
  • Can I add and my own gadgets?
    Yes. You can add and remove gadgets that are listed under the setting page in Evergreen Life.

    In order for Evergreen Life to function correctly, your internet browser will need to be set correctly.

Check the Radio Buttons next to Enable Active Scripting and Scripting of Java applets. Click OK to accept scripting.
